The management of patients with malignant disease has improved significantly over the last thirty years. Advances in surgical and radiotherapeutic techniques and new drug development have resulted in improved clinical outcomes in terms of symptom control, tumour response rates, disease free intervals and survival times. We are fortunate in having a wide armamentarium of chemotherapeutic agents for use in patients with cancer and haematological malignancies. However, the use of these drugs is associated with some morbidity and a small mortality rate. As systemic cancer treatments become more complex it is important to ensure that arrangements are in place to identify and manage drug toxicity quickly and effectively. Chemotherapy is a widely-used treatment for patients with cancer. Patients may receive a number of different courses of chemotherapy; each one consisting of several cycles. For example, one course of treatment could consist of six, three-weekly cycles. The first course of treatment is referred to as "first line chemotherapy". Subsequent courses, usually consisting of different drug regimens, are referred to as "second line", "third line" or "fourth line" therapy. Patients may be treated with radical curative intent i.e. to cure their cancer, or palliative intent i.e. to control tumour growth and or relieve symptoms. Potential side effects of the treatment, such as nausea and hair loss, are commonly acknowledged. However, cytotoxic drugs can also cause serious medical complications that can result in death. Neutropaenia, one of the most common side effects, occurs when white blood cells are destroyed by chemotherapy, leaving the immune system unable to fight infections. Many patients who develop neutropaenia will require a delay in treatment or a dose reduction, and studies have suggested that this can adversely affect treatment outcomes1-2. Systemic infection as a result of neutropaenia can be life threatening. Fever and sepsis during chemotherapy treatment are treated with antibiotics and antifungal agents, which can be toxic and lead to resistance to the drugs. Haematologic toxicity usually occurs at an early stage of chemotherapy treatment. Not only does this affect the present cycle of treatment, but patients commonly experience complications in subsequent cycles1, 3. There are suggestions that around 20 per cent of patients with cancer and febrile neutropaenia a low neutrophil count and a fever ; develop serious complications4, including gastrointestinal infections and severe sepsis. The mortality rate of these patients increases as the duration and severity of the neutropaenia increases. However, the precise mortality rate is unknown in the United Kingdom. In a news article from Stanford University, neutropaenia is reported to be the leading non tumour-related cause of death in cancer patients5. The incidence of febrile neutropaenia varies with cancer type7. The mortality rate of American patients with cancer who have febrile neutropaenia has been quoted as being as high as 8 percent6. Another source reports an overall mortality rate of 4-30 percent for patients with cancer and febrile neutropaenia4. Neutropaenia is just one of several medical complications that can occur with cytotoxic drug therapy. Other examples include anaemia, thrombocytopaenia a reduced platelet count which can sometimes be associated with abnormal bleeding ; , nausea and vomiting, diarrhoea, severe mouth ulceration, cardiotoxicity and renal and liver impairment. The patient may also suffer complications associated with the means of drug administration such as central venous line infections or thromboses and associated pulmonary emboli. Although clinical outcomes are improving, there is concern that the quality of care is not of a consistently high standard across the UK. A number of national clinical guidelines on the management of cancer using systemic anti-cancer therapy have been produced.
