A summary of the DoD P&T Committee's clinical and cost effectiveness analyses are available in the May 2006 meeting minutes. Clinical differences among products within the same subgroups as outlined above ; were minimal. Results of the clinical review and provider opinion supported the availability of a wide array of contraceptive choices. Bottom-line Seasonale, Ovcon 35, Ovcon 50, and Estrostep Fe were selected as non-formulary under the UF because similar alternatives are available at substantially lower cost. MTFs should remove these products from their formularies and either switch patients to formulary agents or establish medical necessity prior to 24 Jan 2007. MTFs should add the new BCF selections to formulary and review products currently on formulary based on relative cost, provider preferences, and clinical needs. The addition of Yasmin, Yaz, and Ortho Tri-Cyclen Lo to the BCF is intended to 1 ; provide uniform availability across MTFs, 2 ; lower the cost of these widely-used products at MTFs, and 3 ; decrease the likelihood that prescriptions will be filled at retail, at substantially higher cost MTFs should carefully select the specific generic versions of generically-available products to be used at their facilities, since prices can vary drastically. The table on the next page shows FSS prices as of May 2006 and postdecision UF Blanket Purchase Agreement BPA ; prices. Particularly note pricing for Lutera, Levlen, Mononessa, Norinyl 1 + 35, Trinessa, and Nor-QD, all of which are generic versions of products on the BCF. ; Specific generic versions of some of the older oral contraceptives are currently available at a lower cost than the brand-name only BCF agents. MTFs may wish to keep these agents on formulary and encourage their use, although careful purchasing is needed and standardizing on a specific generic product may require patient education. Rationale The UF Decision Process All generically available oral contraceptives OCs ; were recommended for addition to the UF due to lower cost compared to brand-only OCs. All alternative formulations transdermal patch, vaginal ring, injection ; were also recommended for addition to ensure adequate clinical coverage. Non-formulary recommendations for specific brand-only OCs were based on cost effectiveness compared to clinically similar alternatives. Special Notes About Seasonale: Seasonale ethinyl estradiol 30 mcg 0.15 mg levonorgestrel ; contains the same active ingredients in the same amounts as Nordette, Levlen, Levora, and Portia, but is specially packaged for extended use 84 days of active tablets, 7 days of placebo tablets. Because conventional 28-day packs contain 21 active tablets, four packages are needed to equal one package of Seasonale. Patent protection for Seasonale expires in September and a generic may become available; however, availability and pricing is speculative at this point. The Committee is likely to reconsider non-formulary status if a generic becomes available at a competitive cost. There is currently relatively little use of Seasonale by MTFs. The BCF Decision Process The BCF selections represent a considerable change from previous BCF listings, most of which were added during the last contraceptive review in 2000. Clinically, it was time for an update; new products are now available and utilization of older products has decreased. From an economic standpoint, generic OC contracts have now run their 5-year course and expired, resulting in recent price increases. Addition of Yasmin, Yaz, and Ortho Tri-Cyclen Lo to the BCF Manufacturers offered BPA prices in return for UF and BCF status that represented a substantial reduction in unit cost at MTFs, which already fill a large volume of prescriptions for Yasmin and Ortho Tri-Cyclen Lo. Basically, cost avoidance could be realized for DoD as a whole either by making these products uniformly available at MTFs primarily decreasing MTF costs and discouraging migration to retail ; or by making these products non-formulary under the UF resulting in higher copays and encouraging switches to lower-cost products, but also incurring medical necessity fees and requiring a large number of patient switches at MTFs ; . UF BPA prices for Yasmin, Yaz, and Ortho Tri-Cyclen Lo are still much higher than specific generic versions of some of the older oral contraceptives. However, MTFs have not consistently purchased lowest cost generic products. This is understandable given complicating factors in this class, including recently terminated contracts, the number and complexity of products, potential for abrupt price changes, and the fact that generic OCs are all "branded" products, complicating patient switches. The Committee concluded that addition of Yasmin, Yaz, and Ortho Tri-Cyclen Lo to the BCF would be the most reliable way to reduce the unit costs actually paid by MTFs. FSS Prices as of May 06 & Post-Decision UF BPA prices are listed on the next page. BPA prices were submitted for three specific versions of generically available OCs, two of which are on the BCF. MTFs are not contractually required to purchase these specific products, but should consider doing so as they are currently the lowest cost versions. Currently, diagnosis of malignant melanoma is made exclusively by excision and consecutive histology. To date no non-invasive method exists to distinguish benign from malignant lesions in vivo. In recent years optical coherence tomography OCT ; has been developed as method for non-invasive in vivo histology. OCT tomograms are based on an interferometric method using low-coherent light with an axial resolution based on the light source's coherence length. At a wave length of 1300 nm a resolution of 15 m achieved with commercially available OCT solutions. Depiction of single cells or dermal cell conglomerates necessary for tumor diagnosis is not possible with these systems. We developed a high resolution OCT HR-OCT ; system using a Ti: Sapphire femtosecond laser with a spectral bandwidth of 80 nm and improved resolution to 3 m with a scan rate of 6 sec. tomogram. To establish feasibility of OCT scanning in melanoma, we performed HR-OCT on excisions of malignant melanoma prior to histological examination. Histology revealed a pagetoid distribution of atypical large melanocytes occurring singly and in tumor cell aggregates forming melanoma cell nests of different size and morphology. HROCT tomograms correlate to histological findings. Melanoma cell nests form homogenous low-scattering structures within the surrounding high-scattering epidermis. At higher resolution, a melanoma nodule in HR-OCT shows a demarcated area of lower scattering within a highscattering area. In the corresponding H&E slide this low-scattering area resembles a nest of melanoma cells surrounded by a ring of atypical melanocytes. Our results demonstrate that HR-OCT is capable of detecting melanoma cell nests in the epidermis and dermis. At present we are implementing a compact real-time HR-OCT setup expecting faster acquisition of tomograms to use HR-OCT as a diagnostic tool in vivo. Overall HR-OCT is a promising tool for future in vivo histology of melanoma providing faster diagnosis and therefore better treatment.
A study on the potential of swamp buffaloes as a source of family income in Central Java, Indonesia, was carried out through a survey conducted from June to July 2001. Buffalo raisers from Brebes, Pemalang, Semarang and Pati regencies were selected to be observed and interviewed. Supporting data for this study were also collected from the government agency for livestock services of Central Java. Data collected were analyzed using descriptive methods. The results indicated that from 1995 to 2000, the buffalo population in Central Java was reduced by 6.48%. The reduction of number of these animals were due to some factors, such as: indiscriminate age of slaughtering, poor calving rate due to limited availability of buffalo bulls to be used for natural breeding and poor reproductive and feeding management. On the other hand, establishing communal shelters found in some regencies was considered as a better management practice for the buffalo production in the province and theo-dur.

Accident RTA ; provides some 30% or more of the average forensic autopsy load to Varanasi region. Chest injuries in combination with the other regional injuries 61.76% ; were the commonest injuries in fatal RTA cases, next to head injuries. The greater frequency of fatality in the regional injuries i.e. chest injuries were due to involvement of two vital organs viz. heart and lungs. Death in case of chest injuries is more often based directly on the effect of asphyxia from disorders of chest function than from associated irrecoverable lesions. Early deaths are caused by hypoxemia, hypovolemia, chylothorax, interstitial emphysema, arterial air embolism etc. MATERIALS AND METHODS The present study comprised of 123 cases which were brought into the mortuary of the department of forensic medicine, Institute of Medical Sciences, Bananas Hindu University, Varanasi, for the medico legal autopsy during the period from march 1st , 2000 to July 31st , 2001.During this study several epidemiological observations and their results have been considered including the details about the type, nature and frequency of injuries to various organs of chest regions mainly and also organs of adjacent abdomen region head and neck etc. OBSERVATION & RESULTS The outcomes of the overall observations and results of 123 fatal chest injury cases have been included in this chapter.
