Table 1 provides the complete 2003 TNM staging system for prostate cancer. The most common preoperative clinical ; stage is T1c or a tumor identified by needle biopsy and detected because of an elevated PSA level. A clinical T2 tumor is one identified by an abnormal DRE result. However, the difference between clinical and pathologic staging is important. As a general rule, pathologic stage is the more important of the 2 because it has been found to be more predictive of ultimate prognosis than clinical stage.114 A true pathologic stage can be obtained only by surgical extirpation of the prostate gland in toto, as with radical prostatectomy. It is important to note the difference between preoperative Gleason score based on transrectal ultrasoundguided biopsy ; and postoperative score based on pathologic examination of the whole specimen ; . Of the 2, the postoperative score is by far the most predictive of ultimate prognosis. PREDICTION OF PROGNOSIS Given the number of variables involved in determining prognosis, a formula is needed to quantify an individual patient's risk of failure after prostatectomy.
