PREOPERATIVE PSA Like seminal vesicle and margin involvement, preoperative PSA has been found to be an independent prognostic factor predictive of biochemical failure after surgery.106, 109, 110 TUMOR STAGE AND GRADE The first staging system introduced was that of Whitmore111 in 1956, which was amended in 1975 by Jewett.112 Coincidentally, in the same year as Jewett's modifications, the first TNM categorization was published.113 In the modern era, the American Joint Committee on Cancer and Union Internationale Contre le Cancersponsored TNM system has been the predominant staging system used. Table 1 provides the complete 2003 TNM staging system for prostate cancer. The most common preoperative clinical ; stage is T1c or a tumor identified by needle biopsy and detected because of an elevated PSA level. A clinical T2 tumor is one identified by an abnormal DRE result. However, the difference between clinical and pathologic staging is important. As a general rule, pathologic stage is the more important of the 2 because it has been found to be more predictive of ultimate prognosis than clinical stage.114 A true pathologic stage can be obtained only by surgical extirpation of the prostate gland in toto, as with radical prostatectomy. It is important to note the difference between preoperative Gleason score based on transrectal ultrasoundguided biopsy ; and postoperative score based on pathologic examination of the whole specimen ; . Of the 2, the postoperative score is by far the most predictive of ultimate prognosis. PREDICTION OF PROGNOSIS Given the number of variables involved in determining prognosis, a formula is needed to quantify an individual patient's risk of failure after prostatectomy. To this end, several mathematical formulas, from complex to simple, have been proposed during the past several years.110-115 Of these, the most widely accepted and validated109 are the Partin tables, which predict extraprostatic extension, seminal vesicle involvement, and lymph node involvement.

The definitions of various subtypes of GA are listed in table 7. Localized GA has solitary or a limited number of skin lesions that may affect one or several anatomical areas5 and papules may be arranged in an annular or arciform pattern.29 Generalized GA, according to Dabski K et al, 25 has widely distributed lesions of a large number affecting the trunk and at least one extremity. Tan HH et al has used the term disseminated GA to describe extensive lesions confined to one anatomical site or to the extremities only.4 But this is the only case series, according to the author's knowledge, classifying disseminated GA as distinct from localized and generalized type. The exact number of skin lesions that defined `extensive' was not mentioned in their article and most other case series in the literature did not include a disseminated subtype.3, 9, 10 Therefore this subtype is not employed in the present study. Subcutaneous2, 24 and perforating27 subtypes differ from typical GA by the level of skin involvement: subcutaneous layer in the former and superficial dermis as well as epidermal involvement in the later. This pathological difference translates into difference in clinical characteristics of skin lesion as outlined above, for instance, rusperdal and diabetes.
