The differential diagnosis of exercise-induced dyspnea in children and adolescents includes EIB, gastroesophageal reflux, hyperventilation syndrome, exerciseinduced anaphylaxis, and cholinergic urticaria. In EIB, dyspnea and wheeze are maximal following 5 to 10 min of exercise.11 Expectoration of mucus is common. Physical examination will reveal increased use of abdominal muscles to assist expiration. Hyperinflation is present and can be confirmed on a chest x-ray film. PFT results, especially after challenge, are diagnostic. The symptoms of heartburn, dysphagia, and bad taste in the mouth during exercise preceding the stridor suggest gastroesophageal reflux. In hyperventilation syndrome, the symptoms are dyspnea, palpitations, chest pain, trembling, and paresthesia.12, 13 Exercise-induced anaphylaxis occasionally is preceded by ingestion of specific foods like celery, shrimp, and chicken. Manifestations are stridor, syncope, choking, colic, nausea, and vomiting preceded by exertion-related pruritis, generalized urticaria, and cutaneous angioedema.14 The hives are large, usually 10 to 15 diameter.15 Wheezing is rare and evanescent, and PFT results are normal.16 Cholinergic urticaria is preceded by exercise, a hot bath, or fever and is characterized by tiny hives about 1 to 3 diameter.15 Bronchospasm usually is present, and PFT results reflect small airway obstruction.16 Although not rare, VCD is seldom recognized. Highlighted here are the salient clinical features of this commonly misdiagnosed condition. Increased awareness by the physician will help to reduce unnecessary office visits, to avoid prescription of needless medications, and to direct management at the psychologic basis.
Electroconvulsive therapy may in fact be a more useful antidepressant strategy for those patients who cannot tolerate medications or whose cancer treatments interact with available antidepressant medications.
